Getting Started in OnePoint
OnePoint Quality Reports
OnePoint’s comprehensive quality reports aggregate data from multiple sources. These reports offer high-level insights into key performance indicators related to cost and quality and on-demand access to patient needs.
As a Vanderbilt Health Affiliated Network (VHAN) member, you can leverage this information to strengthen VHAN’s mission of providing better patient care while helping us to collectively achieve success in value-based contracts, which leads to increased financial incentive distributions.
Identify patients and stratify them based on risk
Filter patients with chronic conditions, identify preventive care gaps and proactively reach out to schedule an appointment so you can improve their long-term outcomes and control costs related to their care.
View patients with acute events
See which patients had recent hospitalizations and discharges to determine if they need a care management program, patients who have over 30 days of an opioid supply but have no recent primary care provider (PCP) visit, and more.
Understand medication use
Learn which patients have not refilled prescriptions or picked up a medication. Find patients on high-risk or high-cost medications and work with other providers to make adjustments if necessary.
